Early Intervention Service Coordination

Our team-based approach means that in addition to your child’s assigned coordinator, you will also have a team that comprises professionals of other disciplines. Every family has a team that includes representatives from speech and language therapy, occupational therapy, physical therapy, Developmental Special Instruction, and even psychology, and/or social work. We offer in-home and community early intervention services for children and families, as well as therapy clinics for developmental disabilities (TDD). What Does a Service Coordinator Do?

At RISE Pediatric Therapies, we come alongside you to provide life-changing therapy for your child and training and support for parents and caregivers that make everyday life easier. Your EI service coordinator helps in the following ways:

  • Upon referral, the service coordinator is the family’s first point of contact with RISE Pediatric Therapies.
  • The coordinator conducts the initial intake/screening.
  • The coordinator familiarizes the client with our services, including the early intervention program, the family’s rights, our funding, the team-based model, and coaching.

a. The team-based model approach means that although your child is assigned a coordinator, their team also comprises professionals from our other services. For example, your team may also include representatives from speech and language therapy, occupational therapy, physical therapy, Developmental Special Instruction, psychology, and/or social work.

b. Parent coaching is a foundation of early intervention. Our specialists work with parents to discover what is working for the family and what isn’t. We’ll share information and resources to support parents in their child’s daily activities and routines to facilitate progress and also ask caregivers to share information with us about the child and family—because you are the experts on your child!

  • If the child is eligible for services, the coordinator facilitates all meetings, including the completion of the Child and Family Assessment and the initial Individualized Family Service Plan (IFSP).
  • The coordinator conducts any reviews of the plan and contacts the families monthly, ensuring services are going smoothly and passing along relevant resources.
  • When the child is nearing the time of the end of services for early intervention, as early as 2 years and 3 months, the coordinator begins the transition process to ensure a warm handoff to any needed after 3 supports, like developmental preschool, other home visiting programs, or other clinic services- including our TDD services.

Overall, your service coordinator will ease your transition into and out of our program and will be your point of contact throughout the whole process.
