Get Started With Therapy for Developmental Disabilities
At RISE Pediatric Therapies (RPT), our therapy for developmental disabilities (TDD) services include speech and language therapy, occupational therapy, physical therapy services, and certified Dynamic Movement Intervention for children from birth to young adulthood. For your convenience, we work with several insurance providers and also accept private pay. Our therapy clinics for TDD are conveniently located in Mesa and Surprise. Our process has five simple steps.
1. Contact Us
Contact us to determine if we have a therapist available to provide sessions in your preferred location (home or clinic).
2. Obtain a Prescription or Referral
Obtain a prescription/referral for the specific therapy from your child’s pediatrician.
3. Complete the Intake Packet
Complete and return the intake packet along with required documentation, including diagnosis, referral, and any assessments or evaluations. We work with insurance to obtain prior authorization, when applicable.
4. Initial Evaluation
A therapist completes an evaluation of the child to determine strengths, needs, and goals/outcomes. Therapist writes a Plan of Care.
5. Return a Signed Plan of Care
Therapy sessions begin once the referring physician signs the Plan of Care, if required.
