Our process has five simple steps.
1. Contact Us
Initiate a referral through the AzEIP website. Anyone can refer. The list below includes some of our typical referrals.
- A parent can self-refer
- A pediatrician can refer
- A community organization can refer
You will be assigned a contractor in your region, and even though you can request to by serviced by RPT, your request may not be granted.
2. Complete the Screening and Intake Packet
Complete the developmental screening/intake with an assigned service coordinator.
3. Establish Eligibility
A child is found to be eligible if:
- A medical record review shows they have an established condition that qualifies them for eligibility.
- We conduct a multidisciplinary evaluation that determines they have a significant delay in at least one area of development. (Please note, RISE Pediatric Therapies does not diagnose conditions, and we don’t need a diagnosis to treat.)
4. Create an Individualized Family Service Plan
Our team works with your family to create a plan based on your family’s routines and activities and the desired outcomes we’ve identified as a team.
5. Determine the Team Lead
We’ll work as a team to choose the Team Lead and any other team members who will provide therapy in your home and during your normal routines and activities, such as during mealtime, playtime, at daycare or during outings, for example.
